This application and a deposit of $300.00 per person in U.S. dollars by credit card or by check on a U.S. bank or by international bank draft or money order or interbank wire transfer will hold your space. We will request additional information later.

If you would like to secure your space immediately, please call us at 800-800-1775 (our international phone number is 317-884-1815) or fax it to 317-882-4274 and we will hold the space for you. We can hold such a  reservation for five business days pending receipt of the printed and signed registration form and your deposit.
